Ayurveda treatment for dark circles, restlessness, drowsiness and more

I have multiple issues listed below. Kindly advise the medication and food for the same.

Sex: Male. Age: 31. Weight: 68 kgs. I am a Vegetarian.Height: 5 feet 7 inches. Marital Status: Unmarried
Chief Complaint: 1) Semen Discharge in Urine 2) Excessive sweating 3) Alopecia 4) Dark Circles 5) Excessive urine passing 6) Urine dripping even after peeing 7) Restless 8) Drowsiness the whole day
Family History: We have a history of baldness and low calcium
Personal History: I was involved in excessive masturbation practices, took tentex forte/ raricap combination for workouts and used supplements for muscle growth.
Laboratory Investigation Reports: 1) CBC ok with a few exceptions as RBC -> 5.54 mill/mm3 and Lymphocytes -> 46.30, basophil-> 0.00 thou/mm3 2) Vitamin D, 25 Hydroxy, Serum(CLIA) -> 17.80 nmol/L 3) KFT was fine 4) Sonography KUB & Prostrate tests indicate within normal limits 5) X-Ray Chest PA view showed normal 6) X-Ray PNS water view indicated left maxillary sinus appear hazy s/o sinusitis, turbinate appear mildly thickened 7) Blood Pressure: 120/80
Other information: Otherwise a normal, athletic guy with a stress free attitude?
