Dear Doctor: Dear Sir or Madam,
I have many problems when I want to study, I can't concentrate, my memory is weak, I procrastinate and when it comes to make decisions I waste time. I often forget things, and can't see the point. In class I sometimes don't have the courage to speak up because I think others will laugh at me. This makes my social life impossible. I get nervous and anxious easily, and daydream. I get easily angry to people I know but am polite to strangers. End of January I have exams. So that's my priority! If that works I am willing to get treated by you.
